What Consent Means
Before getting pierced at Siren Body Piercing, every client must complete and sign a piercing consent form. This form confirms that you understand the procedure, the risks, the aftercare, and that you are choosing to be pierced voluntarily. For minors, a parent or legal guardian must complete additional sections to give consent where permitted by law.
The consent form is separate from this Consent Info page and must be filled out in person at the studio before any piercing is performed.
Who Can Be Pierced
Laws and studio policies limit who can receive certain piercings and under what conditions.
- You must provide a valid, government‑issued photo ID showing your date of birth to be pierced as an adult.
- For minors, a parent or legal guardian must be present with their own ID. In addition, they must provide legal documentation for the minor, such as a birth certificate, school ID, or government-issued ID (such as a passport) that shows they are the legal guardian for the minor. Parent or legal guardian must complete the minor consent section, where allowed by New Hampshire law.
- Clients must not be under the influence of drugs or alcohol and must be in a suitable physical and mental condition for the procedure.
New Hampshire law strictly regulates body art on minors, and some services may not be available to anyone under 18, even with parental consent.
What You Will Be Asked
During the consent process, you will be asked for information that helps keep you safe and allows the piercer to decide if the service is appropriate for you.
Typical questions include:
- Your full name, contact information, and date of birth
- The piercing(s) you are requesting and location on the body
- Relevant health and medical information, such as:
- Bleeding disorders, heart conditions, diabetes, epilepsy
- Immune system issues or conditions that affect healing
- Allergies (especially to metals, latex, anesthetics, or cleaning solutions)
- Pregnancy status where applicable
You will also be asked to confirm that you have had the opportunity to ask questions and that all your questions were answered before you sign.
Risks You Need To Understand
The consent form explains that all body piercings carry risks, and that no procedure is completely risk‑free.
Risks may include, but are not limited to:
- Infection, swelling, bruising, bleeding, or prolonged soreness
- Allergic reactions to jewelry materials or aftercare products
- Scarring, keloids, migration, or rejection of the jewelry
- Damage to teeth or gums for oral piercings, and other site‑specific complications
By signing the consent form, you acknowledge that you understand these risks and still choose to proceed with the piercing.
Aftercare and Your Responsibilities
You will receive written and/or verbal aftercare instructions for your specific piercing. The consent form usually includes statements that you:
- Have received and understand the aftercare instructions.
- Agree to follow all aftercare directions to reduce the risk of infection or other problems.
- Will contact the studio or a healthcare professional if you experience unusual pain, swelling, discharge, or other worrying symptoms.
Proper aftercare is your responsibility once you leave the studio, and it plays a major role in how well your piercing heals.
Minors and Parent/Guardian Consent
If you are under 18, additional steps are required and some piercings may not be available at all under New Hampshire law.
In general, the minor consent process may include:
- The parent or legal guardian being present for the entire procedure (for eligible piercings).
- The parent/guardian providing their own ID and proof of guardianship (such as a birth certificate or court documents) if requested.
- The parent/guardian signing a separate section of the consent form acknowledging the risks and giving permission for the minor to be pierced.
The studio reserves the right to decline service to any minor if required documentation is not provided or if the piercer believes the service is not appropriate
When We May Refuse Service
For safety and legal reasons, the studio may refuse to perform a piercing if:
- You cannot provide required identification or proof of age.
- You appear to be under the influence of drugs or alcohol.
- You are unable or unwilling to complete and sign the consent form.
- Your health or disclosed medical conditions make the procedure unsafe in the piercer’s professional judgment.
- The requested service is not permitted under state or local regulations.
This policy protects you, other clients, and the studio’s staff.
Questions Before You Sign
You should feel fully informed and comfortable before signing the consent form.
Before you sign, you are encouraged to:
- Ask any questions about the procedure, jewelry, healing time, and aftercare.
- Clarify anything in the consent form that you do not understand.
- Tell the piercer about any concerns, anxiety, or medical issues you may have.

11. Limitation of Liability
To the fullest extent permitted by law, Siren Body Piercing, its owners, employees, and agents shall not be liable for any indirect, incidental, consequential, or punitive damages arising out of or related to your use of this Site. Our total liability for any claim related to the Site shall not exceed the amount you paid (if any) to use the Site. This limitation does not affect any liability that cannot be excluded or limited under applicable law.
12. Indemnification
You agree to indemnify, defend, and hold harmless Siren Body Piercing, its owners, employees, and agents from and against any claims, liabilities, damages, losses, and expenses (including reasonable attorneys’ fees) arising out of or related to: (a) your use of the Site, (b) your violation of these Terms, or (c) your violation of any rights of a third party.
